For your convenience, Jurim Dental Studio pays for UPS 2-Day and UPS Ground shipping to and from our laboratory. Upon request, Jurim Dental Studio will provide you with shipping labels as well as all of the necessary packaging materials to properly secure your materials for shipment. Although UPS is our preferred shipper, other options are available. If there are any questions concerning shipping a case, please contact the laboratory at 1-888-6ADRIAN.
1. Complete the Rx Form (Please click here to download)
Complete the Rx Form as thoroughly as possible to allow for our technicians to provide you with the highest quality restoration.
2. Gather the necessary materials
Gather the essential case materials listed below. Each element provides the laboratory with vital information about the dentist’s and patient's goals. Complex cases require another level of preparation and these elements assure the highest quality restorations will be achieved.
- Impressions
- Bite Registrations
- Study Models
- Shade Information (Stump shade and Desired Shade)
- Photos
- Completed RX Form
Wrap and secure the items with bubble wrap or another similar type of packing material. Please ensure that the items are wrapped thoroughly to avoid breakage during the shipping process. Jurim Dental Studio cannot be responsible for impressions and models that arrive at the laboratory broken.
3. Package the Case
Place the wrapped items into a box and secure it with packing tape. Ensure that there is minimal room inside the box for the items to move around.
4. Ship the Case
Place one of the provided ARS return labels on the outside of the box. Please ensure that you complete the top portion of the UPS ARS label with your name, address and zip code. You should peel off the tracking number receipt at the bottom of the label and retain for your records.
- UPS ARS is a prepaid shipping service; no shipping charges will be assessed to you. To ship your package, you may do any of the following:
- Give the package to any UPS driver making a regular pickup or delivery.
- Take the package to any location of The UPS Store, UPS Drop Box, UPS Customer Center or any UPS Authorized Shipping Outlet.
- To find a location nearest you, please visit UPS.com or call 1-800-PICK-UPS. The two nearest drop off locations are included in this letter for your convenience.
- Schedule a special pickup through 1-800-PICK-UPS or UPS.com/pickup. A $5.00 pick up fee will apply for this option if scheduled via telephone, $4.00 pickup fee if scheduled via the web.
